Job Opening: 

Licensed Occupational Therapist

Mary Health of the Sick Assisted Living Facility, Thousand Oaks, CA

Per-Diem or Part-Time

Position Overview

We are currently seeking a licensed Occupational Therapist to join our team on a part-time or per diem basis. The Occupational Therapist will work closely with our residents to assess their needs, develop individualized treatment plans, and implement therapeutic activities to promote independence and overall well-being.

Responsibilities:

• Conduct assessments to evaluate residents' physical and cognitive abilities, functional limitations, and environmental factors.
• Develop customized treatment plans based on assessment findings and resident goals.
• Implement therapeutic interventions to improve activities of daily living, mobility, and fine motor skills.
• Provide education and training to residents, family members, and caregivers on adaptive techniques and equipment usage.
• Collaborate with interdisciplinary team members, including physicians, nurses, and social workers, to coordinate care and optimize outcomes.
• Maintain accurate documentation of evaluations, treatment sessions, and progress notes in accordance with regulatory requirements.

Qualifications:

• Valid license to practice as an Occupational Therapist in California.
• Previous experience working with geriatic populations in a healthcare setting is preferred
• Strong interpersonal and communication skills.
• Ability to work independently as well as collaboratively within a team.
• Commitment to providing compassionate and patient-centered care.
